Two front doors — the Staff Portal and the Patient Portal

commvita gives the people who deliver care and the people who receive it their own self-service front door. The Staff Portal handles the working life — leave, job plan, CPD, declarations, signatures, speak-up. The Patient Portal goes beyond a record viewer: it is proactive (it nudges prevention), it is a safety channel (a patient or family can ask for an urgent review), and it orchestrates a journey — features a personal-health-record like PKB and the national NHS App do not carry — while surfacing through the same NHS Login the NHS App uses.

1 The Staff Portal — the whole working life, self-service

One place for a member of staff to run their own employment admin and governance — no forms to chase, and every governance obligation (declarations, mandatory training, signatures, speaking up) in reach. Reached at /staff-portal; role-aware, so a board member sees board induction and meetings, a clinician sees the job plan.

My time & pay

Leave · job plan · balance

My Leave Balance (annual / TOIL / sick / study / parental), My Requests (book & track leave), and My Job Plan (PA sessions, submit for approval) — with a Team Absence Calendar and the Leave Policy to hand.

Where in commvita /staff-portal · API /rostering · /job-plans.

My learning & growth

CPD · appraisal · passport

My Training & CPD shows statutory & mandatory compliance (RAG), CPD records, appraisal and a training passport — the same data commvita Learn governs. Offline learning syncs from the mobile app.

Where in commvita /staff-portal · API /training · /learn.

My governance & voice

Declare · sign · speak up

Register of Interests (declare & track conflicts), My Signatures (commvita Sign queue & history), My Board Meetings (papers, packs, actions) and Speak Up — a confidential Freedom to Speak Up route straight to the Guardian.

Book leave and see the team at a glance

Request leave against a live balance, see it approve, and check the Team Absence Calendar before you book — so cover is visible, not a surprise.

2

Declare an interest — and sign what needs signing

Raise a conflict-of-interest declaration into the governed Register of Interests lifecycle, and clear your commvita Sign queue (board packs, policies, letters) with an audited OTP ceremony — all without leaving the portal.

Speak up, safely

A confidential Freedom to Speak Up disclosure routes to the FTSU Guardian — named, confidential or anonymous — from the same place staff manage everything else.

2 The Patient Portal — proactive, not just a record

Reached at /portal with NHS Login (OIDC) or a one-time code, and proxy access for a carer or parent. The home screen is not a filing cabinet — it opens with what the person should do next: a preventive nudge, an abnormal result to acknowledge, a message to answer.

Everyday care

Appts · meds · results · messages

Appointments (book, request, reschedule), medications and repeat requests, test results with abnormal flags, secure two-way messages, and a shared care plan.

Where in commvita /portal · API /portal/appointments · /portal/medications · /portal/test-results.

Proactive prevention

Nudges · goals · vaccinations

Preventive Nudges — a rule engine (QOF-register-aware) surfaces overdue/due-soon vaccines, screening, reviews and health checks with Book / Remind me / Decline (decline reason captured for governance). My Goals tracks weight/BP/exercise/smoking with progress; My Vaccinations issues a certificate.

Where in commvita /portal · Home · API /portal/nudges · /portal/vaccinations.

Safety & control

Urgent review · privacy · portability

A 24/7 request for an urgent review by the critical-care outreach team if the person or their family feel things are getting worse; a Data & Privacy hub (who accessed the record, special-category control, opt-out) and a FHIR R4 self-export (GDPR Art.20 portability).

Open on what matters — and act

The home screen leads with a preventive nudge or an abnormal result to acknowledge, and lets the person book straight from it. The portal is a place to do, not only to read.

2

Ask for help when it's getting worse

A clearly signposted card lets a patient, family member or carer request an urgent review from the outreach team, 24/7 — a safety escalation channel, wired to the platform's Rapid Review workflow, not a message that sits unread.

3

Own your data, and your journey

See who has accessed the record and on what basis, control special-category sharing and national opt-out, export a FHIR bundle, and manage linked (proxy) accounts — while the Concierge orchestrates transport, travel and referrals behind the scenes.

3 Beyond a record viewer — what the Patient Portal adds over PKB and the NHS App

A personal-health-record like Patients Know Best (PKB) is excellent at what it does — patient-controlled aggregation and sharing of a record — and it surfaces inside the NHS App, the trusted national front door for appointments, prescriptions and the GP record. commvita is designed to sit with that model and add the layers a record viewer does not: proactivity, a safety channel, action and orchestration.

Fair comparison. PKB genuinely leads on patient-controlled record aggregation and consent, and the NHS App is the right national identity and front door. commvita does not replace either — it uses the same NHS Login and can surface through the NHS App, then adds the proactive, safety and orchestration layers a record viewer is not designed to carry.

How this enhances the NHS App

The NHS App stays the national front door; commvita makes the room behind it deeper — same login, richer content, and the ability to act.

Same identity, no new login

commvita authenticates with NHS Login (OIDC) — the same identity the NHS App uses — and integrates through the NHS App API (appointments, prescriptions, records, messages, notifications), so the person is not asked to learn a second account.

NHS Login OIDC/nhs-app

From read-and-notify to act-and-book

Where the national baseline mostly shows and notifies, commvita lets the person book from a nudge, request PIFU, submit PROMs and raise an urgent review — write-back and action, not just a view.

Book · PIFU · PROMs/portal/rapid-review

One record across settings

commvita carries a cross-setting record — GP, hospital, community, mental health, social care and pharmacy — so what the person sees is not only their GP record but their whole journey, with the source of each entry shown.

Why this matters for a demo. The Staff Portal turns employment and governance admin into self-service with a live audit behind it. The Patient Portal is proactive, safe and orchestrated — it nudges prevention, gives a patient or family a way to ask for help when things are getting worse, and moves a journey along — using the same NHS Login as the NHS App, so it enhances the national front door rather than competing with it. Non-SaMD: guidance, safety-netting and admin; clinical decisions stay with clinicians.
